THE ORIGINAL INSPECTION WAS DELETED DUE TO SYNCING ERROR WITH DHD. THIS REPORT WAS CREATED USING FIELD NOTES.
THIS FACILITY HAS CLOSED AND HAS BEEN CLOSED FOR A FEW MONTHS. THERE IS A NEW MANAGEMENT TEAM THAT IS TAKING OVER THE BUSINESS. THEY NO NOT PLAN ON REMODELING THE FACILITY, BUT THEY ARE PLANNING OF CONDUCTING A DEEP CLEAN AND MAKING ARTIFICIAL REPAIRS. IN ADDITION, THEY PLAN ON BRINGING IN NEW EQUIPMENT. CHANGE OF OWNERSHIP DOCUMENTS PROVIDED. |
